Healthy Home Program Coordinator
The Program Coordinator position is a part-time
position with benefits. The Program Coordinator will work with a team to help
residents of senior housing achieve wellness goals.. The Program Coordinator
will also be responsible for developing Community Health Action Plans for each
site that addresses such issues as fall prevention, medication adherence,
chronic disease management, and transportation. Successful candidate will
enjoy working in a team, and have experience coordinating services, conducting outreach,
and documenting work. Licensed Social Worker or Provisional eligible.
Coalfield Family Services is an administrative service organization that currently provides socially necessary services, mental health counseling and DUI classes. Our home office is physically located in Fort Gay, West Virginia but we offer services throughout the state of West Virginia. We currently have job openings for family support workers, therapist and case managers and the details on those jobs can be found below.
Family Support Worker: requires a high school diploma or GED, clean driving record, ability to pass a state, federal and abuse neglect background check. Position is considered a 1099 contracted worker which means you would be considered self-employed and responsible for your own taxes. Pay is based on the service being provided but ranges from $10-$14 per hour with $.40 per mile when driving to and from clients home. The bulk of the work is done afternoon, evenings and weekends and the hours are based on agency need and those of the client. Family support workers will be responsible for doing transports, and home supervision and supervised visits. Average yearly income of $30,000.
Case Manager: requires a bachelors degree in counseling, psychology, social work, almonds or education, sociology, criminal justice, or related degree. They must be able to pass a state, federal, and abuse neglect background check as well as have a clean driving record. The position is considered contracted 1099 work which means you would be considered self-employed and responsible for your own taxes. Pay is based on service being provided that ranges from $12-$30 per hour the bulk of the services being $20 per hour. Writer also gets $.40 per mile when driving to and from a clients home and the hours are mostly afternoon, evenings and weekends and are based on client and agency needs. Case managers would be responsible for doing supervised visits, in-home supervision, transport, teaching parenting and adult life skills and responding to crisis response is as needed. Average yearly income of $45,000.
Therapist: Requires a Masters degree in counseling, psychology, or social work and the appropriate license for their degree. Supervision can be provided for those with an MSW and all others would be required to be independently licensed. They would need to be able to pass a State, federal and abuse neglect background check as well as having a clean driving record. The pay is based on service being provided that ranges from $30-$40 per hour but there is no reimbursement for gas. The hours are based on client and agency needs.
